Subject: ADULT P&T Approved Stop Dates

Good afternoon Pharmacists, 

The Pharmacy & Therapeutics Committee recently approved pharmacists to place automatic stop dates on the following orders for ADULT patients:

  • 3 day stop date for azithromycin 500mg orders with the indication of community-acquired pneumonia or COPD exacerbation
  • 5 day stop date for oseltamivir orders

The best way to enter these stop dates is to enter the number of doses required to complete the course. This will ensure that a patient does not miss any doses if they are in a procedure, off the floor, etc. Also, the new dot phrase (,duration) of "Automatic duration adjustment per P&T" has been created to place on the orders. This will help to inform providers of the new policy if they are wondering why the medication was stopped. 

Please note, that if a physician puts in a duration on the order then the pharmacist is NOT to change it (physician order trumps P&T policy). 

This is a great opportunity to promote antimicrobial stewardship and appropriate duration of therapy at the time of order verification. Please let AMT know if you have any questions!
