TOC Consult to Pharmacy Process

Good Morning All,

 

As mentioned in Huddle yesterday, we have developed some guidance for the most appropriate way to handle ‘Consult to Pharmacy’ tasks for home medication histories by TOC.  Please use the attached document to guide your workflow when presented with a ‘Consult to Pharmacy’ for a home medication history by TOC in the future.

 

Your role and associated workflow processes are outline briefly below and in more detail on the attached document:

 

  • Verifying Pharmacist:

o   Void inpatient order generated by the ‘Consult to Pharmacy’ (the dummy order) so it does not appear on the MAR

o   You *MAY* notify TOC – however, they do monitor the consult list regularly from 7a-8p so this is not necessary unless you feel the situation is urgent

  • TOC Pharmacist:

o   Create an admin note on the task stating that ‘TOC is aware’ of task

o   Mark task as complete once medication history has been completed and reviewed

o   Void out task to prevent the task from reappearing daily

o   Notify the provider that ordered the consult if it is an urgent issue

 

Please plan to use the workflow outlined above and on the attached document even if it appears TOC has already completed the medication history.  There are instances where TOC may appear to be finished with the history, however, there is additional information pending that should be addressed before the task is marked as complete.
