Subject: Cerner Formulary Update at HH Main: Vancomycin 1 g IVPB Effective Today

Due to a critical backorder of Add-vantage fluid, we have switched to a ‘single-line item’ entry for vancomycin 1 g IVPB at HH Main (including HBH/OPM) to accommodate loading of premix 1 g IV bags in Preop areas (Main OR, CV, Sivley Tower).

 

Going forward, new orders placed in Powerchart at HH Main locations for vancomycin 1 g will now link to the ‘single line item’ product, and the current IV Set is no longer available.  If you are entering an order directly in MedManager for vancomycin 1 g, please select the ‘single line item’ product as you do today for the 1250 mg and 1500 mg entries. NOTE: Current orders for the 1 g IV Set (5 g vial + PSP Diluent) do NOT need to be updated to the single line item entry.  Please let us know if you encounter any issues with this change.  Thank you.
