Subject: Intrathecal Chemo oer Rad Nursing

Going forward, intrathecal chemo orders entered by rad nursing will be reviewed and evaluated by the "Onc" pharmacist. If the "Onc" pharmacist is preoccupied with another patient care issue, the "Onc" pharmacist will contact the UB4 pharmacist to review and verify the intrathecal chemo order.

The intrathecal chemo order will come up in the "Q" PPM, the "Q" pharmacist will then notify the "Onc" pharmacist to address. Please work together as we figure out this process. Let me know if you have any questions or issues.
