Good morning Pharmacists!

We will be going live with meropenem extended infusion ADULT dosing on October 5th. Extended infusion dosing (EID) applies to ALL doses received inpatient except those in the ED, Peri-OP/PACU, and other procedural areas (traditional 30-minute infusions will still be used here). In the table below you will find the new meropenem EID protocol including the infusion times as well as renal dose adjustments:

Unlike with Zosyn, the intervals will not change when patients are on meropenem EID so it will be especially important to review the meropenem infusion duration to ensure the patient is receiving EID. Patients who are already receiving meropenem at the time of the go-live can continue on traditional dosing, but all new orders for meropenem will need to be placed using one of the PowerPlans created. The "ED Meropenem (MERREM) Orders" and "ID Meropenem (MERREM) Orders" PowerPlans have been created in order to help with this transition. Pediatric PowerPlans have also been created (ED PEDS Meropenem (MERREM) Orders and PEDS Meropenem (MERREM) Orders), but these PowerPlans will include traditional dosing as pediatrics is not going live with EID yet.
