HH Main and HHWC Pharmacies:

 

Due to the poor availability outlook of sodium bicarbonate 8.4% 50 mL and dextrose 50% emergency syringes, we will begin removing both from ADULT code trays tomorrow (Friday, May 5th). The process of removing these syringes from crash carts will take several days. We will also, during code tray refilling, begin removing the sodium bicarbonate syringes and replacing the dextrose syringes with D10W 250 mL bags.

Instructions for refilling ADULT code trays (with sodium bicarb and dextrose syringe changes) :

  1. Pharmacy will place a sheet in place of the sodium bicarbonate syringes instructing that syringes can be found in the pharmacist's code kit or in select Pyxis machines (see attached).
  2. The dextrose syringes will be replaced with a D10W 250 mL bag (a pink sticker, see attached, will be placed on the outside wrapping of the bag to instruct that one bag = one dextrose syringe.
  3. The round metal tags that hang from the crash cart drawer handle will have a sticker on them indicating that the sodium bicarb has been removed from the tray (see attached).

At this time, we are not removing these from the PEDIATRIC code trays.
