After bringing this rule live, it was found this alert was incorrectly presenting at time of verification of a discontinued order.  To reduce this noise, we’ve remove ‘Discontinued’ status from rule review while we troubleshoot a solution.  The rule will still alert when comparing to Completed/Voided with Results statuses.

Subject: Vancomycin Order Alert: NEW 8/25/21

 

In order to address previous medication safety reports of duplicate vancomycin administrations, effective today there is a new rule which has been added to Cerner for vancomycin.  Specifically, when an order is verified/entered within MedManager, Cerner will evaluate the start date/time of this new order and compare it to any previous vancomycin orders in a Completed/Discontinued/Voided with Results status.  If the new order’s start date/time is within 12 hours of the previous order’s start date/time, then the alert shown below will present in Med Manager prompting the pharmacist to review previous orders/MAR administrations for proper timing of the new vancomycin order.

 

Important Notes: If a vancomycin order is currently ACTIVE and a new order is placed, the pharmacist will continue to receive Duplicate therapy mCDS window alerts and this alert will not present.  Additionally, per request, pediatric/neonatal nursing units are excluded from this rule.
