Subject: Inpatient Ordering of Slow Fe: Dosing Changed to Elemental Iron Amount (45 mg)

Due to reported confusion regarding conversion of home medications to inpatient orders, effective today, Slow Fe (ferrous sulfate slow release ER Tabs, MedID: 17676) will be dosed in amount of elemental iron (45 mg) instead of total ferrous sulfate (listed as either 137 mg or 142 mg).

Please note this change only impacts this Slow Fe ER Tablet product.

Cerner sentences, formulary, and synonym descriptions have been updated along with Pyxis formulary.  BD Logistics description will be updated later today.  Current inpatient orders have been converted to this new 45 mg dose going forward.
