Subject: Important Note When Performing IV-to-PO Changes

When performing IV-to-PO changes, please be sure that an appropriate Route is selected based on the product/med type.  For example, for a Medication Type order (without an infuse over time or infusion rate—ex. Protonix), please make sure you select “IV Push” as the route as appropriate. 

 

When “IV” is selected as the route instead of “IV Push” in the instance of Protonix, it causes reporting issues associated with smart pump programming as it is considered a ‘Continuous’ frequency type that should be pump programmed.

 

Reference for type for IV routes:

Medication Type: IV Push, IV Bolus

Intermittent Type: IV Piggyback, IV Syringe

Continuous Type: IV, IV Syringe-Cont
