04/24/17 AW -- TOC Update (current units)

Subject: TOC Update (current units)

Good afternoon,

TOC continues to expand almost weekly.  Our TOC technicians and LPNs are currently performing admission histories on the following units at HHM: 3E/W, 4E/W, 4N/NW, 5E/W, 7W, 7N/NE/NW and will expand to 5MST beginning tomorrow, 4/25.  We are also "live" for admission histories on 4S (Peds) at W&C.  Our technicians and LPNs sign into their assigned units via Aionex each day (currently M-F, 7a-7p) and are listed as "HCS Med Rec - ADM".

As expected, we are only "live" with discharge med recs on 7N/NE/NW and 7W (and 5E/W, depending on pharmacist availability) at this time.

If you have a question about anything done by TOC, our patient documentation can be found under the Interventions tab in HCS.  Simply pull up your patient in HCS, select the Interventions tab in the top right quadrant, and read any comments associated with the HCS Activity "TOC Admit" (or "TOC Discharge" if that's the case).  You may also page the assigned tech or LPN via Aionex as mentioned above.

Please let me know if you have questions or if I can assist in any way.

Thank you,
April

Supervisor, Transitions of Care
Department of Pharmacy
