Pharmacists,
 
Today we will begin implementing a trial protocol that will engage pharmacy earlier in the tPA use process. A new condensed patient assessment criteria will now be implemented by Neurology CRNP's during stroke alerts in the emergency department, which will help quickly identify if a patient is a candidate for tPA. The goal of this criteria is to improve our door to tPA times when treating ischemic stroke.
 
The CRNP will place an order in Cerner and call PSP to notify them of the need for tPA. This trial is taking place April 7th – June 7th, and only applies to stroke alerts in the emergency department at Huntsville Hospital main campus. The delivery process of tPA remains the same. 
 
Fortunately, there is minimal impact on pharmacist workflow:
  • PSP Pharmacists will need to keep the box in which the alteplase is supplied in, until it is confirmed the drug was administered (documented on MAR summary, or call and confirm if you wish)
  • In the event that it is not given after it is compounded, the NPs are aware they need to return it to PSP (the drug and the box it came in are required for manufacturer refund)
  • If the drug is returned to PSP, call 5-6263 or email me (aaron.henslee@hhsys.org) and I will obtain the product and begin the refund process with the manufacturer
