Hi, everyone,

 

As previously mentioned in huddle, Pharm IT has created a new pharmacy tag, ",pharm" which will sub to "Order edited by pharmacy,"  for orders requiring pharmacist intervention that do not necessarily "fit" into one of the other tags (i.e. renal, dose per P&T, indication adjustment, etc.).  A few examples I've used it for include the following: filling in infusion duration when it is blank, product selection, changing heparin drips from units/kg/hr to units/hr, making duplicate orders for things like "BID for 7 days then daily" or "x 2 L then stop" in order comments, changing the "LR IVPB once" orders to the actual LR Bolus order sentence (this almost always has to be changed!). These are just a few examples, but this is a quick way to document some of the things we do numerous times a day. I hope this is useful to everyone.
