     System Formulary Restriction:

     Quizartinib tablet orders must be signed by a REMS-certified Hematology/Oncology Attending Physician

     Inpatient use of quizartinib tablets is restricted to patients who meet one of the following criteria:

  • Initiation/new-start treatment for adult patients with newly diagnosed FLT3-ITD AML
  • Continuation of home therapy
