• Restricted to outpatient encounters or clinic use only (i.e., no inpatient use)
• Nusinersen orders must be signed by a Pediatric Neurology Attending Physician
• A benefits investigation must be completed prior to use
• Appeal requests on a patient-specific basis must be made to the Financial Administrative Oversight Committee (FAOC)
