     System Formulary Restriction:

     Lifileucel injection orders must be signed by a Hematology/Oncology Attending Physician  

     Use of lifileucel injection is restricted to patients who meet one of the following criteria:

• Unresectable or metastatic melanoma
• Previous treatment with a PD-1 blocking antibody
• BRAF inhibitor IF BRAF V600 mutation positive 

• Lexicomp
• Micromedex
• UpToDate
• FDA Daily Med
