indicating medication name, strength, and “send to pharmacy for
dispensing” in label comments field.
Please
note: Hospital policy dictates patient’s may only use their home
supply of medication if we are unable to supply the medication while in
the hospital (non-formulary item)
Storage in Pharmacy
All
home meds are to be stored in the pharmacy while patient is admitted
whether or not they are using any of their home meds.
Nursing
is to deliver medication to pharmacy and a “Home Medication”

copy should be scanned and entry “MEDINRX” should be keyed as
a reminder to the nurse that the home medications are being stored in
pharmacy
Controlled
substances should be counted, signed off with RN, and stored in CII vault
under “Patient Home Meds”
Dispensing
Nurse
will request home med via OE system
Debit
1 dose of Home Med (notice no charge is actually made) from meditech and a Talyst label
will print.Pull the
patient’s home medications (from bins or vault) and label
appropriately including expiration, lot #, patient name, and medication
name.Supplies are located in
shelving in the center island near the keying stations.
Release at discharge
Nurse
needs to come down to pharmacy for pick-up.Home medications must be signed out
on the white copy of the home medication form and scanned (again).
Pharmacist
should discontinue “MEDINRX” entry upon processing
Note:
Single doses for dispensing may be tubed to the
floor, but the entire home med supply cannot be tubed
and they must be signed out by RN.
