Outpatient pharmacy provides a variety of services to help
patients in need of assistance.When the outpatient pharmacy is closed the in-patient pharmacy is to
provide discharge medications for patients who qualify.Qualification is determined by Case
Management.
Step 1: Obtain necessary paperwork

(If faxed, original prescription must be obtained)
Step 2: Print labels for out-patient dispensing

Quantity dispensed should be reflective of the

Step 3:
Affix
first label to medication bottle/bagand 2nd to the
back of the original prescription.Then staple prescription, FaceSheet, and Pharmacy Assistance Application and
place in “Outpatient” box attached to CII vault.

“Patient Information” sheets for each medication using
Clinical Pharmacology
Dispense
Patient Information handout(s) and medications(s) to Nurse or House Supervisor
per policy
