Charity Medication Dispensing

 

Outpatient pharmacy provides a variety of services to help patients in need of assistance.  When the outpatient pharmacy is closed the in-patient pharmacy is to provide discharge medications for patients who qualify.  Qualification is determined by Case Management.

 

Step 1: Obtain necessary paperwork

  • Case Management should bring down or fax:
    • Facesheet
    • Pharmacy Assistance Application
      • Be sure name & phone # of case manager is obtained
    • Prescription (If faxed, original prescription must be obtained)

 

Step 2: Print labels for out-patient dispensing

  • Access “Canned Text (Canned Labels)”
    • Mneumonic: CHARITY
    • Enter” to the text field and edit the following information
      • Patient name, DOB
      • Rx # (Current + 1)
      • Drug Name, Strength, Sig, Quantity
        • Quantity dispensed should be reflective of the # days supply they will need until Outpatient opens (usually 2-3 day supply)
      • MD name
    • F12 to File and save edits
  • Access “Custom Reports” from Pharmacist Menu
  • Access “Print Canned Label”
    • Mneumonic: CHARITY
    • Print 2 labels
  • Repeat above process for each medication

 

Step 3:

  • Affix first label to medication bottle/bag  and 2nd to the back of the original prescription.   Then staple prescription, FaceSheet, and Pharmacy Assistance Application and place in “Outpatient” box attached to CII vault.
  • Record information  in  “Charity Notebook After-Hours Log”

 

Step 4:

  • Print “Patient Information” sheets for each medication using Clinical Pharmacology
  • Dispense Patient Information handout(s) and medications(s) to Nurse or House Supervisor per policy

 

Note: The dispensing of scheduled II-V is highly discouraged, but may be considered on a case by case basis.  See full policy and procedure in charity notebook for details
