SMP/TMZ IV orders, dosed by weight

Dosing is based on the trimethoprim component - the IV formulation contains trimethoprim 16 mg/ml.
Since this is a multi-ingredient drug, Cerner cannot handle ordering by mg/kg.
As a workaround, it is possible to order in PowerChart or PharmNet as ml/kg.
