Methadone bulk oral liquid backorder - Convert adult orders to tablets whenever possible. Broselow Tape recall - 2025 editions have been recalled and removed from peds code trays. Nursing units should use the 2019 edition that is stored on their unit when needed. Demerol backorder (6/25) - Vials have been removed from Pyxis machines. Refer to Backorder Guidance Document for more information on alternative options for post-op shivering. Bicillin LA 600,000 unit syringe backorder (9/24): Orders should product assign to the stock solution Doses should be pulled up from the 1.2 million unit syringe in the IV room using PK
