Number of days of Cefotetan will equal number of days of Cefoxitin.
For example: Cefoxitin 1 gm every 8 hours for 3 days will equal Cefotetan 1 gram q 12 hours for 3 days.
Number of dosages will equal number of hours of coverage.
For example: Cefoxitin 1 gram every 8 hours x 9 doses, 72 hours of coverage, will equal Cefotetan 1 gram q 12 hours for 6 doses, 72 hours of coverage.
At the time of order entry, the pharmacist will include an automatic substitution comment which will appear on the eMAR.
