Actions we've taken to minimize mix-up include:
□ Use of TALLman lettering in computers and labels when possible
□ Robotics and bar-coding for medication
□ Admin Rx at JGRC and GMC-D
□ Alerts and pop-up messages in the pharmacy computer and Suremed machines
□ Alert stickers for storage areas/bins in pharmacy
□ Alert messages on MAR and on label of first dose of medication
□ Separate storage bins in pharmacy and Suremed
□ Education and awareness through the SALAD list
□ Review of new drugs added to formulary at P&T includes sound-alike, look-alike issues.
