     System Formulary Restriction:

     Teclistamab orders must be signed by a Hematology/Oncology Attending Physician

     Inpatient use of teclistamab injection is restricted to patients who meet one of the following criteria:

  • Initiation/new-start treatment for relapsed/refractory multiple myeloma
  • Re-initiation of treatment in patients with relapsed/refractory multiple myeloma who have experienced a treatment delay that requires reinitiation with step-up doses
