     System Formulary Restriction:

      Inpatient use of siltuximab injection is restricted to patients who meet all of the following criteria:

  • Orders must be signed by a Hematology/Oncology Attending Physician
  • Cytokine release syndrome (CRS) due to CAR-T in patients who have failed ≥ 3 doses of tocilizumab

     Use for all other indications is restricted to outpatient encounters or clinic use only (ie, no inpatient use)
