     System Formulary Restriction:

     Use of sacituzumab govitecan injection is restricted to patients in outpatient encounters or clinic use only (ie, no inpatient use)

     Orders must be signed by a Hematology/Oncology Attending Physician

     Use of sacituzumab govitecan injection is restricted to patients who meet the following criteria:

  •      Patients who have received at least 2 standard treatment regimens
