     System Formulary Restriction:

      Use of palifermin injection is restricted to patients who meet one of the following criteria:

  • Bone marrow transplant AND undergoing conditioning regimen prior to stem cell transplant that contains thiotepa
  • Undergoing conditioning regimen prior to stem cell transplant that includes myeloablative total body irradiation

      Use of palifermin is restricted to a maximum of 6 doses per course of therapy
