     System Formulary Restriction:

     Mosunetuzumab-axgb orders must be signed by a Hematology/Oncology Attending Physician  

     Inpatient use of mosunetuzumab-axgb injection is restricted to patients who meet one of the following criteria:

  • Initiation/new-start treatment for relapsed/refractory follicular lymphoma
  • Re-initiation of treatment in patients with relapsed/refractory follicular lymphoma who have experienced a treatment delay that requires re-initiation with step-up doses 

     All other use is restricted to outpatient encounters or clinic use only (ie, no inpatient use)
