   Restricted to the following: 

• Outpatient/clinic use
• Inpatient use of dalbavancin injection is restricted to patients who meet one of the following:
  • Antimicrobial Stewardship Program or Infectious Diseases Service approval
  • Patient unable to receive prolonged IV antibiotics and oral therapy is contraindicated or not offered at on-site pharmacies for discharge prescribing
