     System Formulary Restriction:

     Inpatient use of carfilzomib injection (Kyprolis) is restricted to patients who meet all of the following criteria:

  • Multiple Myeloma with bortezomib treatment failure
  • Require urgent inpatient treatment with new regimen (ie, requiring plasmapheresis or dialysis)

     Use for all other indications is restricted to outpatient encounters or clinic use only (ie, no inpatient use)
