     System Formulary Restriction:

      Inpatient use is restricted to pediatric patients who meet one of the following criteria:

  • Initiation of treatment for Classical Hodgkin Lymphoma
  • Treatment of Anaplastic Large Cell Lymphoma and receiving high-dose methotrexate

     Use for all other indications is restricted to outpatient encounters or clinic use only (ie, no inpatient use)
