​​Belantamab mafodotin-blmf (Blenrep) 70 g SDV is RESTRICTED to the following:

• Benefits investigation must be completed prior to use
• Use is restricted to patients in outpatient encounters or clinic use only (i.e., no inpatient use) in patients who meet the following criteria: Relapsed/refractory multiple myeloma who have received ≥ 2 lines of prior systemic therapy, including a proteasome inhibitor & immunomodulatory agent
• Orders must be signed by a Hematology/Oncology Atending Physician
