  • Indication: Treatment of adult patients with relapsed or refractory multiple myeloma after two or more prior lines of therapy including an immunomodulatory agent, a proteasome inhibitor, and an anti-CD38 monoclonal antibody.

  • Prior authorization required: Yes for outpatient
  • Restriction/criteria of use (all will apply unless specified):
    • Relapsed or refractory multiple myeloma who received 2 or more prior lines of therapy, including an immunomodulatory agent, a proteasome inhibitor, and an anti-CD38 monoclonal antibody
    • Radiographically documented disease
    • Negative pregnancy test
    • Eastern Cooperative Oncology Group (ECOG) performance status 0-1
    • Healthcare facilities that dispense and administer ABECMA must be enrolled and comply with the REMS requirements.
