OH Formulary

  • Indication: Prevention of recurrence of C. difficile infection (CDI) in individuals 18 years of age and older following antibiotic treatment for recurrent CDI
  • Service line: Gastroenterology, Infectious Diseases
  • Formulary location: Ambulatory
  • Service location: Clinic, Outpatient procedural area
  • Patient population: Adults
  • Prior authorization required: Yes
  • Restriction/criteria of use (all will apply unless specified):
    • At least 1 recurrence after a primary episode and had completed at least 1 round of standard-of-care oral antibiotic therapy OR had at least 2 episodes of severe CDI resulting in hospitalization within the last year.
    • Current episode of CDI must be controlled (˂ 3 unformed/loose stools/day for 2 consecutive days).
    • A positive stool test for C. difficile within 30 days.
    • Administration will occur 24–72 hours following completion of antibiotic course for CDI treatment.
