Restricted: Adult and Pediatric Hematology - Hematology/Oncology Clinics (pediatric and adult) and System Outpatient Infusion Centers

Criteria for Use:

Diagnosis of sickle cell disease
Age of > 16
VOCs > 2 in 1 year, trial or currently receiving hydroxyurea (received for at least 6 months with 3 months of stable dosing or documented intolerance).
