Restricted: Short acting nifedipine is restricted to OB/GYN.

Recommendations:

  1. Amlodipine is recommended to be used as the 1st line agent. Patients who are on DHP-CCBs other than amlodipine, nifedipine, or nifedipine ER will be therapeutically interchanged to amlodipine while inpatient.
  2. Given safety concerns with nifedipine immediate release, this formulation should not be used regularly. The use of short-acting nifedipine shall be restricted to the OB/GYN service line. This change was approved in 2018.
  3. Due to the infrequent utilization, the DHP-CCBs felodipine, felodipine ER, isradipine, isradipine ER, oral nicardipine, and nisoldipine ER have been removed from the system formulary.
  4. Nifedipine ER will remain on formulary for continuation of home medication when clinically appropriate.
