Dosing Protocols:

****** For oral doses, for patients that can swallow capsules, capsules are the preferred formulation.  Oral solution is available for pediatrics, patients unable to swallow capsules, and patients who have a feeding tube.  Per Medication Orders Policy (900.3233) #16b, pharmacists can substitute oral dosage forms for another form, such as capsule vs oral liquid.
