Inpatient: Non-Formulary medication

All of the following would need to be met for in-hospital use (and the medication ordered in advance through the non-formulary process).

• • Patient is an adult undergoing surgery
  • Patient has a history of OUD or is at high risk for OUD
  • Patient is not taking opioids PTA, and the goal is for the patient to not receive opioids following surgery
  • Cannot take NSAIDs or NSAIDs ineffective
  • Patient does not have a history of significant respiratory, cardiovascular, metabolic, hematologic, neurologic, renal, hepatic, or psychiatric disease
  • Patient is not pregnant
  • Patient is not taking a strong CYP3A inhibitor
  • Suzetrigine will not be continued beyond 14 days
