Rasburicase is Formulary, Restricted to patients meeting criteria and at dosing, as follows:

• Patients undergoing diagnosis for malignancy or at risk for tumor lysis syndrome, who meet 2 of the following criteria:
• Uric acid ≥8mg/dL
• WBC ≥50,000 cells/mcL or high tumor burden as judged by the oncology practitioner
• Serum Creatinine >1.5mg/dL or >1.5 ULN for patient group
• If ≥2 criteria are not met, attending physician should contact pharmacy for consideration of an exception

• Adult Patient Weight	Rasburicase Flat Dose
  Less than 100 kg	3 mg
  Greater than 100 kg	6 mg

• Pediatric Patient Weight	Rasburicase Flat Dose
  Less than/equal to 20 kg	0.15 mg/kg rounded to the nearest 1.5 mg dose
  >20 to 100 kg	3 mg
  Greater than 100 kg	6 mg

• May be redosed once if uric acid (using proper collection technique) remains >6mg/dL at least 12 hours after initial dose and hydration.
