Formulary Restricted, for the treatment of Parkinson’s Disease psychosis, if meeting one of the following:

• Continuation of home therapy

OR

• New starts require treatment failure or intolerance of adequate dose and duration of clozapine and/or quetiapine. Only to be prescribed by or in consultation with neurology or psychiatry. Prior to initiation of therapy, suggest confirming insurance coverage for continuation in the outpatient setting.
