Pemivibart is a Formulary Restricted medication:

• • Restricted to outpatient infusion for patients with moderate to severe immunocompromise
    • Moderate-to-severe immune compromise includes
      • high-dose corticosteroids/other immunosuppressive medications
      • acute leukemia
      • chronic lymphocytic leukemia, non-Hodgkin's lymphoma, or multiple myeloma (regardless of treatment)
      • primary immunodeficiency
      • solid organ transplant recipient, advanced HIV infection
      • actively treated for solid tumor or hematologic malignancies
