Formulary, Restricted for GI clinic use only; criteria:

• • Treatments for at least 2 recurrences (ie, 3 CDI episodes) should be tried prior to offering fecal microbiota transplantation
  • For prevention of recurrence following CDI treatment (administer 24-72 hours following last dose antibiotics for CDI)
  • Age 18 and older
  • Exclusions: baseline chronic diarrhea, IBD, celiac, colostomy, active colitis, previous FMT, require antibiotics for another indication
