Calcitonin injectable is Formulary Restricted to patients with either:

• Baseline serum or corrected calcium concentration >12 mg/dL and presenting with symptoms of hypercalcemia OR

• Baseline serum or corrected calcium concentration >13 mg/dL regardless of hypercalcemia symptom presentation
