Inpatient Formulary Restricted to use by Legacy Transplant Services for: 

• • Continued access for hospitalized patients with a renal transplant already established on belatacept therapy
  • Conversion of hospitalized patients from a Calcineurin inhibitor (CNI) to belatacept when CNI therapy is contraindicated
  • De-novo initiation of belatacept at the time of transplant where CNI therapy is to be avoided or contraindicated.
