Restricted to cardiology providers in the hospital outpatient infusion center through a patient specific prescription managed by Huntsville Hospital Specialty Pharmacy. Each patient request will be evaluated on a case by case basis. Huntsville Hospital reimbursement and insurance certification or PAP eligibility will be reviewed prior to initiation of therapy and a determination made if the prescription can be filled through the Huntsville Hospital Specialty Pharmacy and administered at the hospital infusion center.
