Non-Formulary. Patients may use home supply. Both strengths would be needed to be stocked as the need for dose adjustments may occur. Used for advanced ovarian cancer with BRCA mutations. The patient must have failed 2 previous chemotherapies, rare use expected. Cost is about $500/day.
