Comments:

Zosyn orders for adults are automatically converted to a 4.5 g Extended-Infusion Regimen (4 hours).  (Exception: Doses given in the emergency department, peri-op OR/PACU, or other procedural areas).  Prescribers may elect to continue standard 30-minute infusions by specifying NO SUB.
