SAXENDA (liraglutide for weight loss) is classified as non-formulary, not stocked. Patient may use own supply in the hospital. Defer initiation to the outpatient setting.

 

Victoza is classified as formulary, though patients are encouraged to use their home supply when possible. Initiation of therapy should be deferred to outpatient setting whenever possible.

Victoza is a MULTI-use item. Make sure that only ONE pen is dispensed at a time AND it is TAMPER TAPED. Each pen should be dispensed in a plastic bag with the bright pink (or other neon-colored) label attached that says:

Victoza is a MULTI-use item

Store at patient bedside

Discard Date: __/__/__ (30 days after dispensing)
