Probiotics are restricted to use by GI and pediatric providers. Since probiotics are considered a dietary supplement, if they are ordered by a non-GI/Peds provider, the verifying pharmacist is authorized to discontinue the order.
RISAQUAD is the preferred HH formulary agent (tablet/capsule). Other probiotic products (except for CULTURELLE and LACTINEX granules/powder) should be interchanged to RISAQUAD.
LACTINEX granules/powder are available for tube administration.
Pediatric Automatic Discontinuation: After notifying the ordering provider, pharmacists may automatically discontinue probiotic orders for pediatric patients at HHWC upon confirmation of central venous catheter placement.
