Extina (ketoconazole 2% foam) is non-formulary, not stocked or ordered. Interchange to either ketoconazole cream or shampoo.

Xolegel is non-formulary, do not stock. Ketoconazole cream should be dispensed in its place. (Gel is dosed once daily, cream is dosed twice daily).
