Non-formulary. Inpatients may use their own supply of Afrezza if judged necessary.
Patients who are on Afrezza and are admitted to the hospital should be converted to insulin lispro subcutaneous at a 1:1 unit ratio, same schedule, and titrated to appropriate response.
