Perforomist (formoterol nebulization solution) is classified as non-formulary, not stocked. Interchange to Brovana (arformoterol nebulization solution).

Update:  July 13, 2016 - FORADIL discontinued by manufacturer.  Pediatric patients - contact prescriber for an alternative medication.  Formoterol is still available in combination with budesonide (SYMBICORT).  Arformoterol (BROVANA) inhalation solution is also still available.
